hypertrophy in all chambers....max. in left ventricle......but obstruction occours due to septal(I/V) hypertrophy...which compresses aortic opening in the left ventricle...
but wait the hypertrophy is concentric.....not sure though...so there shud not be any increase in size of heart...i mean overall..no cardiomegaly..
so in a way both are correct....
have to read the topic again.......

Its correct that the Obstructive HCM has a Major Septal Defect and Muscle to Septal ratio in Size is increased. THe Differentiation is done on the Basis of the Septal Enlargement the and the Venrticular cavity the heart size may increase 3Dimensionally therefore the increase will be there but on both sides meaning into the ventricular cavity is also due to in growth of the muscle and septum while the heart size will increase due to LV Hypertrophy and Left Atrial Enlargement.
Obstructive HCM is due to midsystolic obstruction to flow through the LV outflow tract as a result of a the narrowing of the outflow thus dragging the anterior mitral valve movement toward the septum.
The hallmarks of the obstructive type of HCM consist of systolic anterior motion of the anterior mitral valve leaflet and asymmetric septal hypertrophy with a ratio of septal wall thickness to posterior wall thickness of greater than 1.4:1.
A summary of echocardiography findings includes
1. Abnormal systolic anterior leaflet motion of the mitral valve
2. LV hypertrophy
3. Left atrial enlargement
4. Small ventricular chamber size
5. Septal hypertrophy with septal-to-free wall ratio greater than 1.4:1
6. Mitral valve prolapse and mitral regurgitation
7. Decreased mid aortic flow
8. Partial systolic closure of the aortic valve in mid systole.

i remember the heart size can be but is not necessarily increased.
