I found the order of differential in some cases are not the more logic one. For example, in an case with high fever and nuchal rigidity, which is obvious caused by CNS infection (and meningitis is on the top of the list ), migraine is in the 2nd place before other infectious causes (such as encephalitis). Are the items on the list basically possible choice you can pick based on your personal favor or the order really matters?

The mini cases have too many symptoms and hence too many DDs. But on the actual exam the cases are very simple . You can easily reach the diagnosis. Infact you'll have to think hard to come up with DD s ( the cases are soo clear cut!!)

I also noticed that rectal exam is on the top of the workup list in every single case of abdominal pain - is it that important?
