I'm wondering if I take a prelim medicine and then decide to take categorical medicine will this year count? I mean will I take 2 more years or I would still have to take 3 years?

it would take 2 years. but prelim medicine is a competitive field and usually no H1 visas

Depends on you and your performance. And if the program that you are looking at continuing on for Categorical medicine has a second year spot available.

how come that preliminary medicine is competitive but preliminary surgery is not?

1. The workoad is much more in surgery so lots of AMGs perfer to go to IM perilm. 2. it is a definite requirement for some courses, like Neuro, Psychiatry. 3. Some who have chopped liver for IM, hope for PGY2.
