Should we have to counsel for every patient if he/she is diabetic or hypertensive regarding lifestyle modifications or only in relevant cases?

i guess in all patients of dm, htn counselling is must ... short and sweet counselling regarding diet , life style ( exercise) foot care regular medicine and blood tests .........
