This patient has aspiration pneumonia seconadry to his stroke .

I am between A or E but gag reflex is absent so removal of false teeth would ' t help that much

So my definitive answer is E

i was thinking E.it leaves no chance of food being aspirated, although i dont know if its a preffered route

i think it's B

most likely is E....but may not be the most appropriate. So to satisfy the gorillas, pick E

I disagree the patient has pneumonia and the question is for management to prevent requerence and the only way would be with chronic antibiobib profilaxis

  #10

the patient has actually aspiration pneumonia (absent gag reflex ) and the best way to prevent it is not atb but avoid aspiration of food => E best answer

D.

metoclopramide will prevent aspiration by incrsg GI motility.

Although E looks as an extreme option its surves their demand of further prevention..
Another way to look at it is:
metaclopramide will help but will not totally prevent reccurence.
Don't know anything about false teeth
Acid sopression will only decrease acid secretion but will not stop the reflux
Last option is antiobiotic prophylaxis wich may prevent pneumonia but the reflux is still there & u can't just give abs that will cover all the causative organisms
So even by exclusion the ans is E
The only problem is that I don't think we'll have this time luxury in the exam

  #13

Join the above with my (E)
