Patient with multiple injuries, low BP (80/50), rigid abdomen on examination. What is the next step for treatment?
IV resuscitation? or Abdominal exploration?
Which one is the best next step? Please explain. Thanks

UW says that in any pt w/ + peritoneal signs -- nexet initial Mx is Exploratory laparotomy

Kaplan say Fluids 1st for same scenario?

Actually kaplan surgery LN says that first step for patients with blunt trauma to the abdomen and acute abdomen is exploratory lap (pag 7). Of course before surgery it says you have to put bladder catheter, get a large-bore venous line for for fluid administration and give antibiotics if there was a penetrating trauma. But this is not fluid resuscitation, in fact you don't infuse a couple of liters of RL and then decide if operate or not, but just start IV infusion and straightaway operate
