If a person presents with the classic picture of uretric stone,what is the best initial test? 1-Plain abd xray 2- sonogram 3-IVP 4-spiral CT Explain why u choose and why you refuse

NSIM---->X-ray--->If stone not seen, suspecting uric acid or cystine stone---->Helical CT without contrast. USG cannot visualise smaller stones and especially in ureter CT is the best diagnostic test.So, I will go with... Next step-->X-ray-since it is 85% sensitive Best test-->Helical CT

The majority of kidneys stones are radiopaque this includes the most common which is calcium oxalate and struvite which can be easily diagnosed by axr if the stone is radiolucent then you would need to use noncontrast abdominal ct. also check the urine in calcium oxalate and struvite the ph of urine is alkaline and in uric acid stones it is acidic. here is the management calcium oxalate Hydration thiazide diuretic struvite hydration uric acid hydration alkalinize urine
