Hi guys, I need help to cut time in my patient note... I was wondering if I could abbreviate some of the W/U tests so I don't have to write out the whole thing... Please let me know what you think...
LUMBAR PUNCTURE:
Can we just say LP or do we also have to say CSF analysis for culture, gram stain, cell count, protein and glucose. ???
PREGNANCY/Ectopic:
Say Pregnancy test or Urine hCG? or Urine beta-hCG?
JOINT TAP:
Say Joint aspiration or Joint fluid analysis for cell count, culture, crystals, and gram stain???
ELECTROLYTES:
Say Serum Electrolytes, OR have to say Na, K, Cl, Bicarb, BUN, Cr...etc....
Urine Analysis:
Say U/A or UA or have to say U/A for ketones, glucose, microalbuminuria
What do we say for PFTs???
Enough to say Spirometry with lung volume measurements??
Cardiac Enzymes:
Enough to say Cardiac enzymes, or have to say CPK, CPK-MB, Troponin
LFT
I think we have to mention all: AST, ALT, Bilirubin, Alk. Phos.

what I think is it wont matter if you use short forms as physicians will b evaluating the PN
I think I will prefer shorter form like lumber puncture, joint fluid aspiration as there is limited pace in each line if its like PN page in FA.. so better write in that space only.
& if we try to give all details like Lumbar pumcture for cell count, culture, protein, glucose then it won't come in space provided.....

 You are right kity! U/A and Electrolytes are Ok but you have to be specific about others . write related stuff in one line.
Get a few photo copies of the PN and practice writing on them ONLY! VERY IMPORTANT!
