A 68 yr old male presents with progressive dyspneoa, experienced over the past year. By echocrdiography, the left venrticular wall is greatly hypertrophied. His chest radiography reveals pulmonay edema and a prominent left heart shadow. Which of the following conditions probably led to these findings....
