do we have to say all the DD's to pt ?? or ould we just say 2-3 dds to pt but can we write few other dds to in our preference??
and what about our physical exam findings, do we need to tell pt imp +ve & -ve in PE during our closure????

I am not sure you should tell your physical examination findings. You could put like "Based on my findings!".I prefer that

ya i think you are right.. it will save us time... these days I am so concerned about saving time as its $$... afterall time is money

Just say , you told me .....(1-2things fromhistory,)then ,after examing you i think most probably your problem is because of ......but there are other possibilities. So we'll run a few tests mention one or two ......and we'll get back and talk about the treatment.ok?
