A 36-year-old Asian woman comes to the office complaining of dark spots appearing on her
face over the last few years. She initially noticed them after having her first child 5 years
earlier. They got much worse with her second pregnancy 2 years ago. She tried using
over-the-counter bleaching creams, but they do not seem to have any effect. She has no other
medical problems and does not take any medications on a regular basis except for a birth
control pill. On physical examination the patient is in no acute distress and vital signs are within
normal limits. On her forehead, cheeks, and chin, multiple discrete and confluent,
well-demarcated patches of brown hyperpigmentation are present. Complete blood count and
thyroid function tests are normal. After discussing the results of the tests, she remains upset
and asks the physician to explain more about the condition. Which of the following is the most
appropriate response?
A. Another pregnancy most likely would aggravate the skin condition
B. No treatment is indicated, as the lesions tend to fade with time
C. There is no effective treatment and the patient should learn to use coverup cosmetics
D. This is a sign of an underlying endocrinologic disorder and more tests are warranted
E. Use of oral contraceptives is not likely to have any influence on the disorder
