the form asks for immunisation history...titers of a no of diseases with lab reports..is it necessary or shall i fake it? where do u get these titer? about date s of administration of vaccine i have no clear documentation..in fact i have residual paralysis due to poliomyelitis.. i am applying for clerkship..which requires me to fiil up these forms..

Faking it is not the best choice. If they find out you will be in a very deep troubled situation and nobody can help you with that. If you have no clear documentation, you'd better complete the record as soon as you can since residency need that. They will need titer also, for later. You can do titer at your local health department or hospital.

i asked my friend who got clerkship said me..it was shit i can fake it..i am not sure as i dont want to take risk cause i always get into problems easily..but getting titers can costly i think
