If you are considering cancer as dd as your 2nd or 3rd position. Is it wise to say that to pt?? or should we just say we will need to wait till result & just skip cancer.

Hi kity..I think you must definitely say that possibility..I would put like this... I am considering a couple of possibilities. It may be a TB or irritation of the respiratory tract, At the same time, I would like to rule out the possibility of lung cancer since you have few risk factors like smoking, weight loss. To be on the safer side I would like to do investigations for lung cancer also. Since, early detection and treatment is always better!

You can say this: Mr. Smith, after considering your history and physical these are my initial impressions. I am considering the diagnosis of A, B, C, and D. Cancer is also a possibility. To be sure however, we must run some additional tests to rule it out. I would like to do an Endoscopy which is a procedure used to look at your stomach with a camera on a tube. We can discuss the results when I return. Do you have any further questions? If he asks: is it painful? I'm scared. I don't want to have this invasive test done. Say: It may be mildly uncomfortable, but they will give you something to help you relax. It is very important to do this test in order to find out for sure what is going on so we can provide the right treatment. If he asks: Am I going to die? Say: Mr. Smith, please let me reassure you that you are in a very good facility, recieving good care. I will be here every step of the way to help you and give you the best possible treatment. Lets run the tests, and then I will return to discuss the options with you. The key is: 1. Tell the Truth (ALWAYS) -but do it tactfully 2. Give reassurance. 3. Never give false security or lie to the patient.

Yes, you can summarize the positive findings of the H&P if you want. Or mention a few positive findings that stick out. I would just say: "Based on what we talked about, (your smoking habits, and your recent weight loss), and what i found during the physical (some abnormalities with your lungs), i am considering the following diagnoses............."

Thanx a lot Dr virgo. I think its nice way to just show your concern abut Cancer but not going in much detail
