Should you go from head to toe? OR do the most important part first? Does the order matter? Lets say you forgot to palpate and auscultate the carotids when you were doing the CV exam, and now you have moved on to the Abdominal exam and you suddenly remember. can you go back and palpate the carotids after the abdominal exam? Is the patient trained to catch that? Also while writing the note, should the note be written in the order of the physical exam: For example for abdomen we have to 1. Inspect, 2. Auscultate, 3. Percuss, 4. Palpate... So should the findings be written in that order: NO scars, ND, +BS, tympanic to percussion in all 4 quadrants, Tender to palpation in RUQ. sometimes I see it in a different order for example ND and NT are written together.

most of the PEs asked to do on the exam will be a focused PE... you won't have time to do a head to toe assessment... My suggestion - start with the organ system affected, then follow with an abbreviated cardiac and lung exam (i.e patient with stomach pain - full abd exam, auscultate and percuss lung fields, auscultate and palpate heart fields) PN - not necessary to write it in that order... as long as you have positive findings and pertinent negative findings...

yes, cocacola is right. and what if you remember that you had to do some PE, Do it after you are done with whatever you were doing when you remembered, even after the counselling, its ok. Thats the reason why you should not tell the SP after the physical that you are done with the physical, dont say like that, as then you will have kept your option of doing something that you missed, later. you can just say, I also have to check your...... if you dont mind or if its ok with you. I do not know whether they are trained to catch that, but it would still gain you a point on having done that.

i was wondering,if we forget to ask somethin durin our history takin and suddenly remember durin cousellin or during the examination,can we still ask? or is the sequence very important? cos i often remember things as i move along at times

sequence not imp.. that's to help us that we dont miss anything imp.. as long as you are with the pt you could ask Qs. pt is going to mark checklists only.. so you will get that point
