For CS workup,First aid cs puts rectal exam and pelvic exam in two lines as
two different lists but it does put pelvic exam and breast exam as one group.
Can we also put these rectal exam and pelvic exam together as one group so
we can save one list for another test since the maximum list is five tests?

I would say this:
1. Rectal exam and stool for occult blood
2. Pelvic exam and Pap smear
OR
2. Pelvic exam and vagnal swab
3. Breast exam
instead of pelvic exam you could say GU exam or Genitourinary exam.
