inzy 6 yrs. old male child was brought to the physician with the complaints of irritability and lethargy. his grades in the school are worsening.mother complains that he is weak, does not have energy for anything including play.and during play he appears clumsy. her husband works in a battery industry. a diagnosis is made. what is the BEST POSSIBLE investigation modality for this child?
a. CBC
b.basophilic stippling on peripheral smear
c.serum lead level
d.lead lines on wrist and knee films
e.empirical therapy on BAL

wouldnt u want to treat first considering it is a child and there is presentation of cognitive defects

Adults present more with abd pain, foot drop, RTA, anemia

i think the best rx would be oral succimer

the q does say best possible investigative modality. BTW, coars basophilic stippling on a peripheral smear is pathognomonic for lead poisoning.

hey idiopathic...
i think coarse basophilic stippling is finding in any sideroblastic anaemia.
it could be a heriditary deficiency of aminolevulinic acid or alcohol damage to mitochondria....or lead poisoning.
therefore my guess is that , blood lead level would be the most specific test in this case. smiling face
