The 3 yo sister of a newborn baby develops a cough diagnosed as pertussis by nasopharingeal culture. The mother gives a history of having been immunized as a child. A correct statement regarding this clinical situation is:

A. The mother has no risk of acquiring the disease because she was immunized

B. Hyperimmune globulin is effective in protecting the infant

C. the risk to the infant depends on the immune status of the mother

D. Erythromycin should be administered to the infant

E. The 3 yo sister should be immediately immunized with an additional dose of pertussis vaccine

My humble guess is
D. Erythromycin should be administered to the infant

The answer is D because erythromycin can prevent or lessen severity of disease if administered during preparoxysmal stages.

The newborn would have Ab from the mother if the mother had a seconary infection during pregnancy, but in this case she didn't; she is immunized so she has B cells able to readily release Ab against pertussis if an infection occur but this didn't happen in this case

Also hyperimmune globulin against pertussis is ineffective and not recommended; the immunity given by natural infection is not lifelong, so the mother is at risk of getting it; and immunization is unnecessary if a person has culture proven pertussis
