Hello everyone, About a carcinoma case (suspested) any type Eso,colon,stomuch like the one in uw 50 yo with wait loss and gives h/o diff swallowing u'r first impression is ca eso then do we have to tell the pt that it could be cancer or just tel him abt other possibilites??? How do u counsel the pt with highly suspecious cancer first tell other d/d then ca or it could be cancer mr XYZ..or could be other dx...... Please can anybody clear my doubt... Thanks

Hi It would be wise to avoid telling the patient about such a serious disease. You would better tell him that you are about order some additional test before you say anything for sure. cause they can start asking you difficult question, behave badly and you will probably be short of time trying to reassure him I hope this will be helpful

i dont agree wid Moziz...u ve gotta tell the diagnosis to the patient...but keep it last in yr list of d/ds...remember...it is a simulated patient...he will behave badly only wen he has bn instructed to.,...and telling him abt Ca will not matter to him...if he has bn told to b a tough patient,he will be...

You can say this: Mr. Smith, after considering your history and physical these are my initial impressions. I am considering the diagnosis of A, B, C, and D. Cancer is also a possibility. To be sure however, we must run some additional tests. I would like to do an Endoscopy which is a procedure used to look at your stomach with a camera on a tube. We can discuss the results when I return. Do you have any further questions? If he asks: is it painful? I'm scared. I don't want to have this invasive test done. Say: It may be mildly uncomfortable, but they will give you something to help you relax. It is very important to do this test in order to find out for sure what is going on so we can provide the right treatment. If he asks: Am I going to die? Say: Mr. Smith, please let me reassure you that you are in a very good facility, recieving good care. I will be here every step of the way to help you and give you the best possible treatment. Lets run the tests, and then I will return to discuss the options with you. The key is: 1. Tell the Truth (ALWAYS) -but do it tactfully 2. Give reassurance. 3. Never give false security or lie to the patient.
