a 40 male comes with retrosternal and epigastric pain. he attempted suicide 45 minutes ago by ingesting lye. he has pain in his mouth and throat, erythema in mouth and pharynx. indirect laryngoscopy shows erythema of epiglot but without edema, there is tenderness in epigastrium. next best step?
answer is upper GI contrast studies and endoscopy! explanation: because of suspected esophageal perforation.
I'm wondering how we do endoscopy in the case of esophagus perforation! And also I don't know why we should think of perforation in this case?

Lye is a caustic solution which can determine esophageal perforation ( the risk is greater after 48 h); for acidic and basic solution ingestion we should go to UGI investigation- contrast studies ( but not with barium) and endoscopy. http://www.bcm.edu/oto/grand/9293.html

I agree, it is kind of strange because UW in a similar Q teaches to do contrast studies alone, without endoscopy... And Kaplan as well... ___________________ Don't live in a town where there are no doctors
