please, if you know for sure and if there is a reference, not just guess. a 57 yo man has had a MI. Development of which of the following sequelae of the infarction is associated wuth decreased survival? 1. avoidant personality disorder 2. decreased platelet count 3. decr serum catecholamine concentration 4. major depressive episode 5. sexual dysfunction

i answered 5, guess it might wrong. Also remember reading do not remember were that ppl after MI prone to depression and even suicidal thoughts, but 4 sort of too much. also i can't find any reference and worrying about it

Its probably 4. Major deppresive episode that could lead to decreased survival .. whats the story Lucky

i first thought of comorbid conditions that may affect pt s prognosis and influence his care ( such as DM, renal failure, bleding disorder such as heparin induced trombocytopenia wery well known HIT= heparin induced tromb-penia in post MI pts), but after giving 2 nd thought and reading thru some cardio research yes, it is major depress. disorder that is said to be associated with decr. survival rate in post MI pts ! This NBME Qs are very very good for the purpose of clinical line to follow.... I would not care abt my own score on these NBMEs, as long as i get the big line concept behind each & every Qs... and thas somethg that often requires 2-3 "fullofstudies brains"

what about other cardiac risk factors - comorbidities, e.g., diabetes, which is affecting sexual function. ?? what i found about depression and MI http://www.ahrq.gov/clinic/epcsums/midepsum.htm looks like insufficient data.
