Small CC is usully treated w/ Chemo b/c by the time it is Dx its already spread correct? Well, the treatment plan is always Chemo- + Radiotherapy on tumor and mediastinum... Local sugery (something like a lumpectomy in breast cancer) is not given since SCLC is a central cancer araising from/around main bronchi... If surgery is considered, usually the pneumonectomy and bilateral mediastenal lymphadenectomy is preformed... Very long and technically complicated surgery (7-11 hours), and never endoscopic... I am not sure if initial workout includes brain CT/IMR to exclude mets, but if they are found, usually total irradiation of brain cranium is given. Gamma-knife is not used because mets are usually numerous and progression of the disease is rapid...
