Guys , I had look for this UW qs BIG TIME just to post it here so we can clear this concept .

Here we go DOCTORS cool

!

A 22 yo AA male sufferering from Sickle Cell anemia presents to the ER with a sustained painfull penile erection for the last 4 hours . Previously , he has one episode of Acute Chest Syndrome that was treated with oxygenation, hydration and blood transfusion .His Pulse is 76/min , resp 16 /min BP 115/76 Temp 37C . Examination of all his system is unrevealing .

What the the next best step in managent ?

a ) Observation

b ) Hyperbaric oxygen therapy

c ) Exchange transfusion

d ) Surgical intervention

e ) Hydroxyurea

I am firm about IV hydration in case of SS priapism, but this is not an option here to choose from... So, colleagues, I am positive about

And, by the way, look at the Q: previous case was managed by oxygenation, hydration and blood transfusion... I do not believe that hydration was given after transfusion, so the hydration is given first... This was my point in SS-priapism case...

yeah i completely agree with justice cz hydratio has to be first but since its not an option so exchange transfusion i s right...but i fear that on exam if both options would be ther i would choose hydration n the right answer may be exchange transfusion then i may be in trouble rolling eyes
