I think B is more likely - this is unlikely to be a case of bipolar disorder and therefore lithium has no role here. The presence of auditory hallucination and flat affect point to a diagnosis of schizophrenia, and risperidone should be used.

and the pt has potential of hurting himself so-hospitalization

I believe you are wrong this is a psychotic disorder the person is having positive symptoms which are the auditory hallucinations and negative symptoms as well as noted by the flat affect this is Schizoprhreniform disoder and the management is Antipsychotics and is the answers it is risperidone which is an Atypical antipsychotics Big Time
