Haemophilus influenzae and Streptococcus pneumoniae are the organisms most commonly found in adults. In chronic sinusitis, the infecting organisms are variable, and a higher incidence of anaerobic organisms is seen (eg, Bacteroides, Peptostreptococcus, and Fusobacterium species). In children, similar organisms are seen, with the addition of Moraxella catarrhalis. In older children and young adults, Staphylococcus aureus is an occasional finding. In systemically impaired hosts, Candida, Aspergillus, and Phycomycetes may be the cause. Risk factors include the following: diabetes mellitus, cancer, hepatic disease, renal failure, burns, extreme malnutrition, and immunosuppressive diseases. Considering that most of the people are immunized to HIb, I go with (D)

We think Moraxella and Haemophilus more when it starts with a viral URI, with clear nasal drainage, and then complicates with a bacterial infection (worsening of malaise, higher fever, purulent rhinorrhea).

young_doc, now you got me! I know that from long time ago, when I was studying Pediatrics for the very first time, and recently read it again while studying for step 2, but I am not sure where it was. If I find the information again, I'll post it. Worth rememebering: the Haemophilus involved in most otitis and sinusitis is the non-typable, and NOT the one we have vaccine against.
