B - Well fitted footware prevents external injuries which patient doesn't sense due to decreased sensation.
A – Difficult to give answers here as no ECG shown, but using Occam’s razor, I’d go for Atrial fibrillation, since it’s the MC type of arrhythmia in hyperTHs
B - Pustule bursting in to painful leisonn and lymphadenopathy. H ducreyi. Chlamydia lesions are mostly painless.
E – I’d go for Traumatic rupture of kidney. Pyuria should be there for APN as someone has already mentioned. Massive amount of blood needs massive source
C – >10yrs.
A – He has a stressor so either PTSD or Adjustment. Now if the stressor is not that severe it is classified as adjustment disorder.

By the way first question is a straighforward UW question, which makes me wonder if there is some connection between NBME and UW! I have seen quite a few questions posted here as NBME ,coming directly from the UW canon!
