an 80 year old lady with 20 year history of urinary incontinence comesto you with complains of mild exacerbation for 3 months. urinary loss occurs while performing daily activities like shopping or drivng. No relation ship with coughing or sneezing. she underwent appendectomy at teh age of 24 and has one daughter. She takes no medicine . Pelvic exam shows atrophic cervix with out palpable uterus or adnexa. labs show Hb 13, Na 140, Cl 105, K 4.5, HCO 25, BUn 15, Glu 120, Creatinine 1.1. Urinalysis epithelials 5-10, WBC 0-1 hpf, glucose None, Bacteria occasional which of the following is the most likely cause of her incontinence A detrusre instability B hyperglycemia C neurogenic bladder D obstructice uropathy E UTI

I think E is more plausible - the combination of bacteria in urine and urinary incontinence make infection a likely explanation, even though it is not stated in the question whether urgency is present. Correct me if I am wrong.

In urge incontinenece there is loss of urine that occurs in large amounts this is due to involuntary detrussor muscle contractions management is anticholinergic medications
