tibc alwasy goes inversly to ferritin....if ferritin up tibc low....also tibc is low in sideroblasti anemia as ferritin is high the difference between the two is that only serum iron is high in sideroblastic anemia and low in ANOCD...

Let me explain as GULJAN said................... TIBC.... Indirect measurement of transferin ( transferin is iron binding protein ) Normal = 300 mg/dl Transferin synthesis in the liver correlates inversely with iron stores in the bone marrow. TIBC= Transferin .. same thing Now......................... two condition A- Decreased iron store in the bone marrow.... causes increased liver synthesis of transferin: serum TIBC is increased.... eg Iron deficiency B- Increased iron store in the bone marrow... causes decreased liver synthesis of transferin: serum TIBC in decreased.. eg anemia of chronic disease and iron overload state including sidroblastic anemia... so in both condition TIBC is decreased ............
