Officially, no surgery is recommended even for localized forms (i.e. with no proven mets anywhere at the time of diagnostics). Chemo with platimum-based protocols 3 courses + radio (47-52 Gr) on the total lung + total mediastimum + 3 other courses of the same chemo. If Pt has metastatic pleuritis, do drainage and inject doxo - causes pleurodesis (cavity obliteration)... Practically, if you can remove the tumor, give 2 cycles of chemo + radical pneumonectomy with mediastinotomy and lymph node dissection... Then you give the same as above... Can still be confusing...
