u've to remeber that the woman is the patient..the physician needs to make contact with her & create a forum in which she can communicate without any interference from her husband... the best way to do this is to ask to speak to the patient in private .....

Note that, either the woman or her husband may object to this request ....

---> if the husband objects ---> offer to have a female staff member present while he is out of the room

---> if the wife objects ( & she has the right to do so ) -----> then ask what would make her most comfortable ....

The Key here is that not to get caught up assuming that because of the couples religion & culture, they necessarily have certain beliefs or desires. Membership in a group doesn't mean that one shares all the beliefs of that group. To assume other wise is mere prejudice.

Yes the source of the question is important to me, at first the question seemed to be incomplete however it's from the Qbank. Some MCQ's originate from the Internet, sometimes they may not convey a real point. I would have chosen option 1. I'm not quite sure how I would feel if a Dr asked me to go out of his office, I would simply refuse. From my experience in the USA, when my attending used to examine a woman he would always ask for a chaperone(similar to a mehrem), I found out about this because I stepped out of the clinic for a moment before he examined her and he asked me to come in again because of that reason.

It's not clear if the husband is actually interfering with the patients history or symptoms however that is a possibility. One of the other things to take into consideration is Domestic Abuse. The fact that she is a muslim and is wearing a head scarf could be a diversion to throw you off, she could be a victim. I don't think there is a clear cut answer to this question although their answer is 2, I don't think it is the most appropriate one.
