do i need to send form L 2,L5 and transcripts directly from medical school to california board ?
IF NOT ,
i have original transcripts(I PAGE MENTION CLINICAL ROTATIONS) . if i send those originals directly to cal. board do i still nedd to send form L 5?

I think the board only accept form L2-L5 and transcript sent directly from your school. {correct me if i'm wrong}

Bear is right. Only those from medical school are accepted except you can send original diploma to them and they will send back to you. The rest are from medical school.

ya, you have to submit L2 & L5 form. You can call your medical school and they will send you the forms and then you can submit in the medical board.
