A 35-year-old woman with a history of alcohol abuse and depression presents in an obtunded state after an overdose of alcohol, benzodiazepines and a tricyclic antidepressant.

Which of the following represents the most serious threat under these circumstances?

1) Delirium tremens
2) Respiratory arrest
3) Status epilepticus
4) Cerebrovascular accident
5) Cardiac arrest

Resp arrest because alcohol and benzo are CNS depressants. Delirium tremens occurs after a few days due to alcohol withdrawal.

Actually i just read the an overdose of TCA leads to arrythmia and cardiac arrest, I would say in this case cardiac arrest. (benzos and alcohol would lead to resp depression- not sure if it leads to respiratory arrest)

TCA's SE are 3Cs: Coma, Convulsions, Cardiotoxicity. I think i would have chosen #2 because coma + alchool- and benzo-induced resp depression sounds to me as a high risk for resp arrest. But anyway am not sure...
