What is the DOC ( antibiotic) for a pt with valvular heart disease to prevent infective endocarditis ?

the general recommendation is Ampicillin + gentamicin before and amoxacillin after.( vancomycin in penicillin allergy) also I remember reading about the use of second generation cephalosporins but I'm not sure.
