New AHA guidelines do not recommend endocarditis prophylaxis for patients undergoing GU and GI manipulations - there is no proven benefit even if the pt has colonization with Enterococci.
What would you do on test?
Stick to old guidelines?
Accept new ones?

I guess one thing is missing to answer this Q: do these Pts have any valvular pathology? As for absolutely healthy individuals, there is no need for prophylaxis...

Even for the patients with cyanotic disease/prosthetic valves... in absence of infection/verified colonization with pathogens, prophylaxis is not recommended.
But, I've just seen that the paper with new guidelines is being published (so officially available) on May 8th 2007, so it shouldn't affect me since I'm taking exam on April 30th.
