from Kaplan book, it is said: IV vancomycin have no effect in the bowel for it doesnot pass the bowel wall Oral vancomycin have no systemic effect for it also does not pass the bowel ball how to understand the mechanism of vancomycin? thanks

Practically this should mean: if you want to have a local effect limited to gut (C difficile treatment), give it per os with no risk of systemic effect; alternatively, if you need a systemic effect, give it iv with no risk to kill normal gut flora... In both circumstances vamco will not cross the bowel wall...
