This Shud Ring A Bell To US
A 16 year old girl with obesity was referred with abdominal swelling and mild ankle edema. On examination the blood pressure was 140/90 mmHg.
Investigations revealed:
Hemoglobin 10.5g/dL (11.5-16.5),
Serum biochemistry normal,
Serum albumin 34 g/L (37-49),
Urine dipstick proteinuria +.
Which investigation should be performed next?
1) 24 hour urinary protein estimation.
2) Abdominal ultrasound.
3) Plasma protein electrophoresis.
4) Urinary albumin: creatinine ratio.
5) Urinary B-Human Chorionic Gonadotrophin test (B-HCG)

This young girl has been ‘gaining weight’, has abdominal swelling and ankle edema. She is hypertensive and has a mild anemia with proteinuria. These signs should ‘ring a bell’ suggesting a concealed pregnancy with pre-eclampsia. The most relevant investigation would be a pregnancy test – urinary B-HCG.

i think preg test is the best b/c we are asked whats next and ac to dr goljan think cheap about boards 1st so preg test and could be preeclampsia or eclampsia(juz considering preliminary diagnosis,but could anything tho) ...couldnt be... i might eb wrong too in reasoning but ladder rule....
