What is the appropriate mode of colorectal cancer screening for the
following case?

A 44-year-old man whose father died of colon cancer at age 77 and who
is asymptomatic.

(A) Colonoscopy now and every 10 years
(B) Flexible sigmoidoscopy now and every 5 years
(C) Colonoscopy at age 50 and every 10 years
(D) Colonoscopy now and every 10 years
(E) Stool occult cards every year; colonoscopy if positive
(F) Colonoscopy at age 40 and every 5 years
(G) Colonoscopy in 3 years
(H) Colonoscopy in 1 year
(I) Colonoscopy every 1 to 2 years

What's the diff b/w A & D??

Because this man has a first degree relative with colorectal cancer his screening should begin at age 40 or ten years before the relative got it...so that wud be 66. but i'd go with the 40.

this pt. is beyond 40, so option F wud be wrong????raised eyebrow

colonoscopy NOW wud be good but then a 10 yr gap in b/w is too long...

This is (A)... Pts with a family history of CC must be started at 40 (F)... But the Pt is 44, so the answer is (A)

Hey guys,
Funny thing is that there is no correct answer among given... This is one of the Fischer's weird Qs, I just found it, and the correct answer is (F)... The base line is that those who have a family history of CC should be started 10 years earlier than the average population, at 50 years and 40 years, respectively... Thus, I was correct when thought that the correct answer should be (F)... Now, the problem is that that the Pt is 44 yo, and when we consider options, we cannot think retrospectively, but now appears we should... Please make sure you know that and take into consideration... Another baseline is that C. Fischer is a mentally disturbed person... If these Qs are for training purposes, they should not be that much confusing... This is my strong (yet personal) opinion...mad

I think it would be appropriate to write C. Fischer and request bring down the level of unnecessary "difficulty" of his Qs... I am going to do so...

(F) Colonoscopy at age 40 and every 5 years

Explanation:

Colonoscopy is the preferred method of screening for colon cancer.
Average-risk persons should undergo colonoscopy at age 50, and if
normal, every 10 years. If a polyp is found, the colonoscopy should be
repeated after 3 years. When there is a family history of colon
cancer, screening should begin at age 40 or ten years prior to the age
of the family member. The earlier date is respected. Follow-up
examinations for persons with family histories of colon cancer should
occur at 5-year intervals. When there are multiple family members,
screening colonoscopy should be performed at age 25 and every 1 to 2
years (characteristic of persons with hereditary nonpolyposis
colorectal cancer (Lynch syndrome). Colonoscopy is recommended 1 year
after a hemicolectomy for colon cancer to verify the absence of
recurrence and the presence of new lesions.

This IS a stupid question. So what will you say to this patient?
Hey man, Dr. Fischer told us 40 and every five year after that. So come next year?
Patient should have a colonoscopy now and then every 5 year.
