21>A mailman receives a severe bite wound from a pit bull while delivering
mail. The wound is cleansed and he receives a booster injection of tetanus
toxoid and an injection of penicillin G. The bite site becomes inflamed
and purulent. The exudate was cultured on blood agar and yielded
gram-negative rods. Antibiotic sensitivity tests are pending.
The agent most likely to be isolated is
A. Bartonella henselae
B. Brucella canis
C. Clostridium tetani
D. Pasteurella multocida
E. Toxocara canis

29>A previously healthy 18-month-old girl is brought to the office with
2 days of irritability, poor appetite, and pulling at her left ear. She
has no known allergies, and her vaccinations are up-to-date. On
examination, the child's temperature is 102.8 F. She is easily
consoled by the mother and moves her neck spontaneously without
discomfort. There is a clear discharge from the nares. The left
tympanic membrane is erythematous, dull, and bulging. Which of
the following virulence factors is generally absent in the
strains of the causative organism that produce otitis media,
compared with those that produce epiglottitis or meningitis?
A. b-Lactamase
B. IgA protease
C. Lipopolysaccharide
D. Nonpilus adhesins
E. Pili
F. Pneumolysin
G. Polyribitol phosphate

2 - g, bcs vaccine is against capsule, noncapsulated strains still can cause otitis, arthritis, but not meningitis.

I'm not so sure if C is the right answer for number 2. Strep Pnuemo is the most common causitive organism for Otitis Media, but the next two in the list are H. Influenza and Moraxella, both of which are gram (-)ve bacteria and thusly have LPS.....
Since the girl does have all her immunizations, she probably got her H.influenza vaccine....but still.......

1-d
2-g
I always associate dog bites with pasteurella, not sure about B hensalae
Also the forms of H influenza that have a PRP capsule cause the epiglottidis, which is what you get in the vaccine, but you are not vaccinated againt the less virulent strains which lack a capsule and cause Otitis media, because the vaccine is against the PRP capsule. Question: How can you differentiate between P multilocida and B hensalae? I have a hard time discriminating againt the two.

  #11

agree, 1d, 2g
bartonella is more connecetd to cats and cat scratch disease.
Hemophilus Infl, that causes ottits media is usually nontypable (although the major causative agent is pneumococus), HIB is the most common for epiglostitis. and since it is nontypable and the vaccine is mainly against HIB (50%), it is possible she got the infection.

Hope I am right, since my exam is on Monday.

i have already made a mistake. HIB vaccine is 95% effective, but vaccine against naiseria type B is 50%. O God! shaking head

  #17

D and B.

  #18

1...........surely D

2.............has to be G.....polyribitol PO4 is present in organism causing meningitis and epiglotitis in children of age 2months to 2 yrs...that is........... H.INFLUENZA

but is absent in organism causing otitis media......that is STREPT. PNEUMON....

i think that is what asked in the Q........

no chance for LPS.......out of question...wink
