S aureus

Overall, this is the most common cause of IE, including PVE, acute IE, and IVDA IE.

I would say Pseudomonas and Serratia are the most common cause of ACUTE endocarditis in chidren, thinking about nosocomial endocarditis, catheter-related. I think if a child is abusing IV drugs on streets (which I think is the minority of cases, but not completely sure...), she would get S. aureus, the same way adults do.
