A 48-year-old woman complains of a mass on the front of her neck. She states that she sometimes has difficulty catching her breath. Her lips feel tingly, and she says that sometimes her muscles twitch for no apparent reason. Examination of the neck reveals a stone-hard thyroid gland. damn u r Right Salmonlla it can be Riedel Thyroiditis cos the presentations is almost the same but teh Stony Hard Appearence is characteristic for Riedel's

The Answer shud be C. Diffuse fibrosis

i dont think the cause of Muscle twitching will be Hypocalcemia i think it wud be the invasion of the cervical plexus cos her lips tingling n twitching r not in different areas SO maybe i m not so sure on this one but th eStony Hard appearence n resemblence with a Ca of thyroid the other possibility is of Reidel's

Reidel's thyroiditis---> thyroid is replaced by fibrous tissue---> clinically MIMIC CARCINOMA
----> extension of fibrous tissue into surrounding tissue ( can even go to trachea to obstruct, can be associated with mediastinal obstruction...)

Riedel's mimic carcinoma bcs it "stone hard".
my guess it medullary carcinoma, just choices for histo bizarre.

Explanation: Choice (C) is the correct answer. Riedel's thyroiditis is a rare from of thyroiditis that results in dense fibrosis of the thyroid gland that can lead to hypothyroidism. Compression of the trachea or esophagus may result in dyspnea or dysphagia, respectively. Physical exam shows a stone-hard or woody thyroid mass. Riedel's thyroiditis is associated with antithyroid antibodies. Involvement of the parathyroid glands can lead to hypocalcemia.
