I have seen some small variations on other threads...
P - past illnesses, infections, chronic diseases, etc...
A - allergies
M - medications
S - surgical history
H - hospitalizations
U - Urinary tract history
G - gastrointestinal tract history
S - Sleep history
F - Family history
O - OB/GYN history
S - Social History (SODA) - Smoke, Occupation, Drugs, Alcohol
S - Sexual history
you should check digidocs website... lots of mnemonics...
