NON ED
Step 1 - Intermittent on Albuterol for Acute exacerbations
Step 2 - Mild persistent :-Inhaled steroid (low dose), cromolyn, or nedocromil (Children usually begin with a trial of cromolyn or nedocromil.)
Step 3 - Moderate persistent:- Anti-inflammatory, inhaled steroid (medium dose), or inhaled steroid (low-to-medium dose) and long-acting bronchodilator, especially for nighttime symptoms (either long-acting inhaled beta2-agonist, sustained-release theophylline, or long-acting beta2-agonist tablets) (If needed, give inhaled steroids in a medium-to-high dose.)
Step 4 - Severe persistent:- Anti-inflammatory or inhaled steroid (high dose) and long-acting bronchodilator (either long-acting inhaled beta2-agonist and sustained-release theophylline or long-acting beta2-agonist tablets and steroid tablets or syrup long term) (Make repeated attempts to reduce systemic steroid and maintain control with high-dose inhaled steroid.

Buddy in Children its the Cromolyn which is started as Prophylatic while in Adults Salbuterol is the mainstay in 2 ways for severe excrebations we have it as a Nebulized form n then in Inhaler form then u give Corticosteriod based on th excrebation or the presentation of the pt Mostly ppl who have Allergic Asthma or Rhinitis they r now being benefited by Leukotriene Inhibitors and is given when they have been around in the round allergen. Usage of Ipratropium is now basically used in th eER Setting not Long term effects have been seen.
