A 50 year old woman presented with a recently discovered, solitary, thyroid nodule. Which of the following would suggest a diagnosis of thyroid malignancy? 1) Elevated serum thyroglobulin concentration. 2) Features of thyrotoxicosis. 3) Ipsilateral Horner’s Syndrome. 4) Previous iodine 131 therapy. 5) Tenderness over the nodule. 3? -iodine 131 has not been shown to be a human carcinogenic. -A malignant nodule doesn't present tenderness -If I understand it right serum thyroglobulin is used in the followup and not in the diagnosis -Thyroid malignancy are most often cold nodules

The association of Horner's syndrome and a thyroid nodule would suggest invasion of the sympathetic chain and would suggest that this thyroid nodule is malignant. Previous I131 is not associated with the development of malignancy. Thyroglobulin may be elevated in any thyroiditis. Tenderness over the nodule would suggest a thyroiditis and thyrotoxicosis suggest a functional adenoma making the malignancy extremely unlikely
