A 28-year-old woman, who is 29 weeks' pregnant, is evaluated because of progressive dyspnea of 3 weeks' duration. Cardiovascular examination shows elevated jugular venous pressure and a heart rate of 100/min. The apical impulse is prominent, and a parasternal impulse is present. An opening snap and a grade 2/6 diastolic rumble with presystolic accentuation are noted in the fourth left interspace. No systolic murmur is noted. S2 is accentuated. Mitral stenosis is the answer, based on Echo and the descroption of the murmur.
