This is from 2007 USMLE sample CD - I have no idea what it is. The answer is shown. Could anybody explain??

the dude has signs of wet beriberi due to chronic thiamine deficiency due to alchol consumption.. all those red wines that he takes daily.. that has caused him to have the heart failure shown by the low ejection fraction.. and coronary arteries have no evidence of atherosclerosis so exercise and other stuff causing protection from atherosclerosis are not much needed right now.. so stop alcohol is the correct option..

Yup in Absence of Atherosclerosis Alcohol is the most likely cause of his conditon nothing else wud help him Guys he is 6 ft tall he supposed to be in th erange of more than 86kg.
