A 7-year-old boy tires easily while running. On auscultation, a harsh continuous murmur is heard at the left of the sternum between the first two ribs. Arterial blood oxygen content is slightly higher in the right hand than in the left hand. X-ray films of the chest show no abnormalities. Which of the following is the most likely diagnosis? (A) Anomalous return of pulmonary blood to the right atrium (B) Patent ductus arteriosus (C) Patent foramen ovale (D) Stenosis of the aortic valve (E) Tricuspid atresia

nice challenge Epica... 1 suggestion though... do create a new topic to review things, this way one can easily click directly on that one and not accidentally find it ( as in my case) Lets see Coronary circ already consumes lots of O2 under N cond---> AV diff is high Renal& splanch. b c is overperfused---> high venous Po2---> low AV diff Pulm is a very compliant circuit therefore an incr in Press--> vessel dilation---> so i think there is not much of press differences during exercise versus N conditions---> about 10 mm Hg ---> low AV diff

1. Splanchic circulation will constrict--- blood flow to tissue will decreased---tissue need now O2---will be extract of oxygen from the blood --- decreased venous po2 -----INCREASED A-V PO2 AS A RESULT try to explain another two

for coronary circ.. the extraction is already MAX under N cond---> therefore no increse VERSUS N resting cond.. depends how you word the Qs... i answered it( increased) as if there was no comparission to N sit. Splanchnic--> low AV diff. resting and exercise as well Pulmonary.... since is a very very compl circuit... why would this differe in exercise ??? in exercise pumping more blood is successfully adjust it by compliance nature.... yes , the extraction might increase, but no much press differences from N sit...

actually in Splanchnic& renal arteriolar Resistance INCREASES DURING EXERCISE---> so less blood flowing in , now these tissues starve O2 therefore more O2 extraction from the blood itself----> higher AV difference.. agree on this one

Yes, because of constriction in Splanchic, clear now
