A 50 year man presents with lightheadness. He has frequent nonsustained ventricular tachycardia on the ECG and cardiac monitor. His bloods show a
Hb 13.0 g/dl,
WCC 7 x 10^9/l,
platelets 230 x 10^9/l,
urea 11mol/l,
creatinine 80μmol/l,
sodium 134 mmol/l,
potassium 3.2 mmol/l,
serum magnesium of 0.6 mmol/l (0.75).


Which one of following is likely to be responsible for his arrhythmias?


A. Poor diet
B. Alcoholism
C. Frusemide
D. Diarrhoea
E. Hyperphosphataemia

Answer: c) frusemide. The likely cause of the arrhythmias is hypomagnesaemia and hypokalaemia, which is most commonly associated with diuretic use.

Ya true i dint notice low K also but in alcoholic also we commonly see low mag levels and ca also(not indicated here tho)b/c of malnutrition, malabsorption, diarrhea, and Mag and ca contribute to alcoholic tetany and cardiac arrhythmia so answered but simply ignored pot levelssticking out tongue was bit tricky
