cyanosis of lower distal extremities,no cyanosis in proximal extremities, NO MURMURS, Loud S2. 21 yrs of age. options : PDA, COPD, ASD, 2 more choices which were clearly not the answer.

may be the ans is pda

it seems preductal coarctation of the aorta were the lower part of the body is perfused by poorly oxigenated blood coming from the pulmonary artery via the patent ductus arteriosus... BUT :

Two things bother me:
-patent ductus arteriosus is associated with a "machinery " murmur
- the age. Usually preductal coarctation of the aorta don't survive the neonatal period without surgical correction.

u meant peripheral cynosis not central?or juz lower limbs(extremities) u mentioned ?if is the 1st case thn COPD if not thn plzz can u plzz rewrite question for me??

ok...here what i learnt...

PDA is a left to right shunt
when the pressure in the pulmponary vessels increases and the flow reverses...there is a stage when there is a net balance between the left to right ,...and the right to left shunts.....this is when there is no murmur...the only indication for pulm hypertesion is a loud P2

later the right to left shunt predominates...eisenmengers

keepgoing a friend gave me this qs as is
so we havet to conclude the answwer from these same wordings
she told coarctation was not the choice

and cynosis and clubbing is only in lower limb not upper limb when reversed flow is present,due to severe pul HTN..!
