A 26 yr old has multiple comminuted fracture of both femurs secondary to motorcycle accident,on his 2nd day in ICU,he devolps alterations in his mental status,petechia,and dysnea.his arterial saturation is decreased but PTT and PT are normal.what is teh mechanism most likely responsible for this patients clincal condition?

1.DIC

2.pneumonia

3.fat embolism

4.atelectasis

5.pulmonary embolism

Fat embolisation follows fracture of shaft of long bones (most commonly femur) or severe soft tissue trauma and is most likely mechanism behind this patients clinical condition...

also fat embolizim takes its time ! its not instantaneous but can take 1-2 days ! some one please explain petechia here? i hope its not thrombocytopenia due 2 fracture (never heard anything like this)

large fat droplets are released into the venous system. These droplets are deposited in the pulmonary capillary beds and travel through arteriovenous shunts to the brain. Microvascular lodging of droplets produces local ischemia and inflammation, with concomitant release of inflammatory mediators, platelet aggregation, and vasoactive amines.

It takes abt 12- 36 hrs for the entire disease to go into ARDS

fat embolism and NOT DIC guys as fat embolism leads to thrombocytopenia and petechia
