24 years old newer army recruit, was brought with complaints of headache and seizures. seizures were sudden in onset, generalised tonic clonic seizures and assoicated with incontinence. on examination, patient was febrile,103F ,and has neck rigidity. brudzinski sign was postive. a diagnosis of meningitis was made and physician was informed that his immunization status isNOT COMPLETE. which isthe most likely agent to have caused the illness in this child? a.streptococcus pneumoniae b.haemophilus influenzae c.group B stereptococcus d.neisseria meningitidis e.escherichia coli f.listeria monocytogenus

Neisseria...MC in age 1-18, and very common in crowded locations (dorms, barracks, etc.). he is older, but the history is better for Neisseria.

H.influenzae is most likely. If there is similar problem with other soldiers n.meningitidis

Neisseria Meningitidis is the correct answer. I really dont understand why such a straight forward simple question is missed by some people. As one of the students correctly pointed out, this is THE most common agent in the given age group, and more so in crowded conditions.That his condition of not being fully immunized does not matter here.There is always some sort of ambiguity in board questions , and they do test how confident you are about your factual information. Adenovirus in choices should have made life difficult ,but then I DID NOT INCLUDE THAT. congrats to those who got it correct. Sorry, I should have been more responsible , and should have posted the answer a lot earlier than I actually have. (Author had completed his residency in Pediatric medicine,and worked as consultant for nearly 5 years)
