The GP was called to the nursing home to see a 75-year-old man with dementia and severe pruritus. On examination, he had excoriations over his trunk and limbs. There was some scaling over his palms, most prominently in the web spaces. Which is the most likely diagnosis?

Iron deficiency anaemia may cause pruritus but rarely a skin rash.

Uraemia in chronic renal failure may cause pruritus but rarely a skin rash.

Diabetes is associated with several skin conditions but doesn’t usually present with pruritus and a rash.

Scabies infestation is common within institutions and typically causes a scaly rash on the hands with burrows and scaling in the web spaces.

Atopic eczema doesn’t usually predominate in the web spaces
